The Ketogenic Diet Is No Light Undertaking: It’s a Complete Hormonal Shift

Fatigue, palpitations, insomnia, dizziness: what’s called adaptation is sometimes a poorly prepared terrain.

The ketogenic diet is not a passing food trend. It represents a profound shift in the body’s energy system. When carbohydrates are drastically reduced, it’s not just a food group that’s removed. The body’s hormonal command changes. Insulin drops, the kidneys release more sodium, water follows, blood pressure can fall, the brain must learn to use ketones more efficiently, the liver increases endogenous glucose production, and mitochondria reorganize their energy production pathways.

To present this as a simple “diet” is a mistake. The ketogenic diet can be powerful. Precisely because it is powerful, it can also be misused.

When Insulin Drops, the Body Changes Its Logic

The first shift is insulin-related. Fewer carbs mean less glucose to manage, so insulin demand decreases. For someone with insulin resistance, this can be liberating: fewer spikes, fewer crashes, fewer cravings, less permanent fat storage. But this drop also changes sodium and water management.

Many symptoms attributed to “sugar withdrawal” are actually signs of poor electrolyte adaptation: fatigue, headaches, dizziness, palpitations, weakness, a hollow feeling, and decreased performance. Simply drinking more water is often not enough. The body doesn’t operate on pure water alone but depends on gradients of sodium, potassium, magnesium, and circulating blood volume.

A beginner entering ketosis without understanding this may believe they can’t tolerate fat when they simply lack salt. They may think they need carbs when they are actually experiencing plasma volume depletion. They might consider the ketogenic diet dangerous when in fact they have applied an incomplete protocol.

Cortisol Can Turn Adaptation into Survival Mode

The second shift involves cortisol. The ketogenic diet requires the body to produce glucose via gluconeogenesis and mobilize more fat. In a well-nourished, well-salted, well-rested individual, this transition can occur smoothly. But in someone stressed, undernourished, anxious, insomniac, or already exhausted, the same protocol can trigger a survival response.

Cortisol rises to maintain blood sugar. Sleep fragments. The heart rate accelerates. The person wakes too early. They feel “wired” yet tired. Sometimes they mistake this tension for energy. In reality, they are operating under stress.

This is where many go wrong. They think they are adapting when they are actually compensating. The body does not calmly switch to fat metabolism; it grits its teeth.

Ketogenic, Carnivore, and How You Function

The Athletic Carnivore approach does not impose the same transition on everyone. Someone already low carb, stable, active, and sleeping well can enter a ketogenic or carnivore phase quickly. Someone coming from a standard diet, with high stress, sugar intake, snacking, anxiety, or chronic fatigue, may need a gradual progression.

This is not about lacking discipline. It’s about respecting biology. Some people must first stabilize protein intake, increase salt, improve sleep, reduce coffee, simplify meals, and only then lower carbs further. Others require a stricter reset to eliminate trigger foods. The right protocol is not the one that seems most radical but the one the nervous system can truly sustain.

Thyroid and Performance Are Not Slogans

Another trap concerns the thyroid. On low carb or ketogenic diets, some markers, especially T3, may shift. This doesn’t automatically mean disaster. It can reflect an energy adaptation. But in someone already fatigued, undernourished, cold, constipated, lacking libido, and with poor recovery, a decline in thyroid function must be taken seriously. Context is key.

The same applies to performance. Ketosis can improve endurance in certain adapted profiles but may impair explosiveness if the transition is poorly managed, electrolytes are insufficient, calories are too low, or the athlete confuses restriction with optimization.

Thus, the ketogenic diet is neither miraculous nor inherently dangerous. It is demanding. It reveals the underlying terrain.

The real question is not: “Can I do keto?” The real question is: Is my body ready to switch fuel sources without turning this transition into chronic stress?
