Promises abound everywhere: to make hemorrhoids disappear with a cream, suppository, fiber supplements, sitz baths, or a miracle cure. Relief can indeed be real. But for many people, the problem returns. Sometimes more painful, sometimes more inflamed, sometimes accompanied by bleeding, pressure, or a burning sensation. What if the issue wasn’t just local but systemic?

Hemorrhoids are not initially a disease. They are normal vascular structures of the anal canal. They contribute to fine sealing and continence. They become pathological when they become congested, inflamed, dilated, bleed, or thrombose. In other words, the problem is not their existence. The problem is the underlying condition that puts them under pressure.

Focusing solely on calming the area without understanding what sustains the congestion is like emptying a bathtub without turning off the tap. The cream acts on the symptom. Meanwhile, the underlying condition sometimes continues to send the same message: pressure, inflammation, stagnation, irritation.

Mechanical Pressure, Inflammation, and Microcirculation

Three mechanisms dominate. The first is mechanical. Chronic constipation increases intra-abdominal pressure and local pressure during evacuation. Repeated straining, hard, bulky, or difficult-to-pass stools traumatize the tissues. Prolonged sitting, sedentary lifestyle, and lack of movement can also slow venous return and worsen congestion.

The second mechanism is inflammatory. A diet rich in sugars, flours, processed foods, oxidized oils, alcohol, or irritating foods in certain individuals sustains low-grade inflammation. Vascular walls become more reactive. Mucous membranes tolerate friction less well. The anal area, already highly vascularized and sensitive, becomes a visible outlet for a more global condition.

The third mechanism is metabolic. Chronic hyperinsulinemia promotes sodium and water retention, alters microcirculation, and often accompanies broader vascular inflammation. Elevated cortisol disrupts digestion, sleep, tissue repair, and abdominal pressure through muscle tension and stress. Leptin and ghrelin indirectly contribute by modulating hunger, snacking, weight, inflammation, and energy stability. None of this is visible in the mirror, but all influence the tissues.

The Pitfall of Automatic Fiber Recommendations

The classic advice often involves adding fiber. Sometimes this helps. But for many profiles, especially those who ferment, bloat, produce a lot of gas, or have an already pressured gut, a sudden increase in fiber can worsen the problem. More fiber sometimes means more volume, more fermentation, more distension, more effort, more abdominal pressure.

This is where the carnivore or low-carb logic becomes interesting. By drastically reducing fermentable carbohydrates, cereals, legumes, certain irritating vegetables, and industrial products, the fermentative load often decreases. Less fermentation means less gas. Less gas means less pressure. Less pressure means less strain on the hemorrhoidal plexuses.

It is also important to correct a common misunderstanding: going less often to the bathroom does not necessarily mean constipation. A highly absorbable animal-based diet produces fewer residues. If evacuation is easy, painless, effortless, and without a sensation of blockage, a lower frequency may simply reflect less waste. True constipation is difficulty, not just spacing.

Stabilizing Insulin, Reducing Irritation, Improving Hydration

A diet focused on animal proteins, well-tolerated natural fats, salt, water, and simple foods can act on several fronts simultaneously. It reduces glycemic spikes. It decreases repeated insulin stimulation. It improves satiety. It removes some dietary irritants. It sometimes reduces bloating. It helps some people lose belly fat, which also lowers abdominal pressure.

Hydration must be understood correctly. Drinking more water alone is not always enough. In low-carb or carnivore diets, insulin reduction often leads to increased sodium loss. If salt is too low, stools can become drier, pressure can increase, and the person may believe the diet “constipates” when the transition is simply poorly managed. Water, salt, natural fat, and sufficient food quantity all matter together.

Fat also plays a role. Too little fat can make transit dry and sluggish. Too much fat, for some, can irritate or accelerate transit. The right amount depends on the individual. Again, it’s not about following a dogma but reading the signals: hard stools, burning, urgency, bloating, pain, heaviness, sensation of incomplete evacuation.

How Long to See a Difference?

It is important to distinguish symptomatic relief from deep correction. Pain, burning, and inflammation can sometimes decrease within a few days if intestinal pressure, fermentation, and dietary irritation drop quickly. Mild bleeding can also improve in this window when local trauma decreases.

Venous congestion, however, often requires more time. One to two weeks may be enough to feel a clear change if diet is the central factor. Older, thrombosed, very large, or fibrotic hemorrhoids may require more time and do not always respond to dietary correction alone. Honesty is key: the underlying condition matters greatly, but local anatomy also exists.

In the long term, the goal is not just to “get through a crisis.” The goal is to reduce the conditions that make a crisis likely: constipation with straining, excessive fermentation, inflammation, abdominal overweight, sedentary lifestyle, stress, poor sleep, and a diet that sustains hunger and snacking.

The Local Symptom Often Speaks for the System

Hemorrhoids remind us of something mechanical medicine sometimes forgets: the anal canal is not isolated from metabolism. Microcirculation, connective tissue, abdominal pressure, digestion, hydration, inflammation, and the autonomic nervous system converge in this area.

The question is therefore not just “which cream to apply?” The question is: why is this area under pressure? Why do the tissues react so much? Why is transit difficult? Why does the gut ferment? Why does circulation stagnate? Why does the body send this symptom to this precise spot?

When the underlying condition is corrected, the symptom often loses its reason to return. Not always immediately. Not always completely. But often enough for the question to deserve serious consideration.
