What Is the Carnivore Diet? Origins, Biological Mechanisms, and Persistent Myths

The carnivore diet intrigues, fascinates, and divides opinion. Sometimes hailed as a radical metabolic therapy, other times dismissed as a dangerous nutritional deviation, it consists, in its strictest form, of consuming exclusively animal-based foods: meat, offal, fish, eggs, and occasionally dairy products. No plants. No fruits. No tubers. No plant-derived carbohydrates.

But behind this seemingly simple definition lies a more complex physiological reality. The carnivore diet is not merely a list of allowed or forbidden foods. It represents a profound shift in biological fuel. For some, this shift demands a complete reorganization of lifestyle.

A Shift in Biological Fuel

Historically, this approach follows the lineage of low-carbohydrate and ketogenic diets. It rests on a straightforward hypothesis: modern diets, rich in refined carbohydrates and ultra-processed foods, disrupt insulin regulation and promote inflammation, insulin resistance, and metabolic diseases. Eliminating carbohydrates would thus restore a more stable hormonal environment.

Biologically, the near-total removal of carbohydrates leads to a drop in basal insulin levels. Blood glucose stabilizes. Lipolysis increases. Fatty acids released from adipose tissue become the primary fuel. The liver produces ketone bodies, which notably fuel the brain. The body enters a state dominated by lipid oxidation.

This metabolic shift is real. It partly explains the improvements reported by individuals suffering from obesity, type 2 diabetes, metabolic syndrome, or certain inflammatory diseases. Lower insulin means less fat storage, reduced glycemic variability, and less inflammation triggered by repeated blood sugar spikes.

But physiology is not limited to blood sugar.

Myths Clinging to the Carnivore Diet

The first myth surrounding the carnivore diet is that of inevitable deficiency. The argument is intuitive: without plants, how can one meet vitamin, mineral, and fiber needs? Yet animal products, especially organ meats, boast remarkable nutritional density. B vitamins, active vitamin A, heme iron, zinc, selenium, and essential fatty acids are present in significant amounts. The question of vitamin C, often brandished as proof of impossibility, is more nuanced: in a low-carb context, relative needs may be reduced, and small amounts found in some animal tissues might suffice. This doesn’t mean every carnivore approach is risk-free, but biology isn’t as binary as public discourse suggests.

A second myth concerns the kidneys. A high-protein diet is supposedly harmful to kidney function. In individuals without pre-existing kidney disease, current data do not show that high protein intake causes renal failure. The transient increase in glomerular filtration rate observed is a physiological adaptation, not evidence of structural damage. However, in people with chronic kidney disease, caution is essential.

The most heated controversy involves cardiovascular risk. Some strict carnivores experience significant LDL cholesterol increases. Others see triglycerides drop and HDL rise. The situation is heterogeneous. The pathophysiology of atherosclerosis involves apolipoprotein B-containing particles, including LDL. Ignoring this fact would be unwise. But reducing cardiovascular risk to a single number without considering insulin, inflammation, blood pressure, glycemic profile, and vascular imaging is equally simplistic. Specific long-term research on strict carnivores remains limited, and uncertainty demands individualized monitoring.

Another rarely discussed point in enthusiastic circles concerns physical performance and lifestyle. A metabolism oriented toward lipid oxidation is remarkably efficient for moderate-intensity endurance efforts. But explosive, repeated glycolytic efforts—sprints, team sports, very intense and intermittent physical work—largely depend on muscle glycogen.

A strictly carnivorous individual, with no carbohydrate intake, can maintain glycogen stores through gluconeogenesis. However, this endogenous production is not unlimited. Certain physically demanding professions, high-intensity sports, or highly intermittent work rhythms may become harder to sustain without strategic adaptation.

In these cases, two options emerge. Either the individual adapts training and work pace to a predominantly lipid metabolism, favoring endurance and intensity management. Or they choose to introduce targeted carbohydrates occasionally, accepting no longer being strictly carnivore but remaining within a low-carb metabolic framework.

Cardiovascular Health, Kidneys, and Performance: Moving Beyond Slogans

Here the debate matures. Is the carnivore diet a rigid identity or a physiological tool? For some, total plant elimination is therapeutic and non-negotiable. For others, the goal is metabolic stability, and methods can be adjusted according to life constraints.

Finally, perhaps the most persistent myth is universality. The carnivore diet is neither a universal panacea nor a systematic aberration. It is a powerful metabolic intervention. Like any intervention, it interacts with genetic background, physical activity, stress, sleep, and environment.

The question is not just “What is the carnivore diet?” The question is: in what biological and social context does it fit?

Adopting an exclusively animal-based diet is not merely a food choice. It is a fuel change. And sometimes, a lifestyle change.

The carnivore diet forces a rethink of the relationship between energy, work, sport, and health. It is not just about what’s on the plate. It engages an entire physiology.
