I Treat My Gout with Red Meat

For decades, this statement was considered medical heresy.
Gout? Avoid red meat. Avoid organ meats. Avoid anything containing purines.
And yet, an increasing number of people today tell the exact opposite story. They eat more red meat. Their uric acid sometimes rises. And their gout attacks disappear.
This paradox shouldn’t exist. According to modern nutrition, gout is simple: too many dietary purines produce too much uric acid, uric acid crystallizes in the joints, inflammation explodes. So less meat means less gout.
That’s the theory.
But real biology is far less obedient.
Uric Acid Doesn’t Come Only from the Plate
Nearly 70% of the uric acid in the blood doesn’t come directly from food. It comes from the normal turnover of our own cells. DNA and RNA constantly break down, releasing purines that the liver then converts into uric acid via the enzyme xanthine oxidase.
In other words, even in a world without steak, uric acid would still exist.
Diet is just one player among many.
Yet red meat remains the prime suspect.
The problem is that large epidemiological observations tell a more complex story. It’s not just heavy meat consumers who have the most gout. Sugary drinks, fructose, and alcohol play a major role in triggering the problem.
Fructose, a Uric Acid Accelerator
Fructose is particularly interesting.
When it enters liver cells, it is phosphorylated by fructokinase. This reaction rapidly consumes ATP and produces AMP, which is then quickly degraded into uric acid.
It’s a direct metabolic accelerator.
A glass of soda can trigger a much greater uric acid production than a steak in an already disturbed metabolic context.
But the mechanism doesn’t stop there. Insulin also plays a central role. Chronic hyperinsulinemia reduces the kidney’s ability to excrete uric acid. The kidneys become less efficient at eliminating it. Uric acid accumulates in the blood.
Gout then becomes less a problem of production and more a problem of elimination.
Why the Carnivore Diet Can Change the Terrain
This is precisely where the carnivore paradigm becomes interesting.
When carbohydrates disappear from the diet, insulin drops. The kidneys can start eliminating uric acid more efficiently again. Insulin resistance recedes. Systemic inflammation decreases.
In the first weeks of a ketogenic or carnivore diet, blood uric acid may even temporarily increase. Ketone bodies use some of the same renal transporters as uric acid, creating transient competition.
This phenomenon is often interpreted as a worsening. But a few weeks later, levels often stabilize or even decrease in some people.
This detail is rarely explained.
Elevated Uric Acid Doesn’t Always Mean a Gout Attack
There is another, even more troubling paradox: a high uric acid level doesn’t necessarily mean gout.
Most people with hyperuricemia will never develop an attack. Monosodium urate crystals must form, deposit in the joint, then trigger an intense immune response involving macrophages and activation of the NLRP3 inflammasome.
Without a favorable inflammatory environment, these crystals can remain silent.
In other words, uric acid is a necessary but not sufficient condition.
In a metabolic environment marked by insulin resistance, oxidative stress, and chronic inflammation, conditions become ideal for these crystals to trigger an immune storm. But when systemic inflammation decreases, blood sugar stabilizes, and insulin drops, the biological terrain transforms.
The same uric acid concentration may then no longer provoke an attack.
What if the Steak Isn’t the Culprit?
This phenomenon is observed in many people adopting a very low-carb diet. Their uric acid levels sometimes remain high, but attacks disappear.
Modern medicine remains cautious about these observations. Controlled studies are still limited, and gout remains a multifactorial disease involving genetics, kidney function, hydration, and overall metabolism.
But one thing is becoming hard to ignore: for decades, red meat has been blamed as the driver of gout, while metabolic mechanisms increasingly point to another suspect: fructose, chronic insulin, and metabolic dysregulation.
Red meat, on the other hand, provides complete proteins, glycine, creatine, heme iron, and B vitamins. Elements the body uses to repair, produce energy, and maintain structure.
Maybe the steak was never the problem.
Maybe the true trigger of modern gout lies elsewhere: in a food environment saturated with fast sugars, sugary drinks, and chronic hyperinsulinemia.
How long have we been treating gout by cutting out meat, while never really addressing the metabolism that causes it?
The Terrain Matters More Than the Isolated Food
Recent research on gout and hyperuricemia forces us to move beyond overly simplistic thinking. Dietary purines exist, of course, and organ meats or certain fish can increase precursor load. But recent mechanistic reviews also show that fructose rapidly increases uric acid production in the liver, while insulin resistance reduces its renal excretion. This double movement—producing more and eliminating less—better describes modern gout than simply blaming steak.
This is where the carnivore approach becomes interesting: it doesn’t “cure” gout by magic, it mainly removes the major modern metabolic accelerators. It eliminates sugary drinks, desserts, liquid fructose, snacking, flours, sweet sauces, and repeated insulin spikes. It changes the context in which uric acid circulates. Blood levels may remain elevated for a phase, but inflammation, blood sugar, insulin, and energy stability change simultaneously.
Caution remains necessary: kidney function, hydration, electrolytes, alcohol, history of attacks, and medications strongly affect individual situations. But continuing to point to red meat as the main culprit while ignoring fructose, insulin, inflammation, and kidneys is treating gout as a disease of the plate, when it is often a disease of the metabolic terrain.
What if the real question isn’t “how many purines do you eat?” but “in what hormonal, inflammatory, and renal state does your body have to manage these purines?”
