During extreme physical effort, the body does not prioritize digestive comfort. It prioritizes mechanical survival. Running, climbing, cycling, pushing, sustaining high intensity for extended periods—all demand a brutal redistribution of blood flow. Active muscles become the priority. The heart and brain remain protected. The intestine, however, is relegated to the background.

This is one of the blind spots in performance. Many athletes believe digestive troubles stem solely from a poorly timed gel, a meal eaten too close to exercise, insufficient hydration, or a sensitive gut. These factors certainly matter. But the core mechanism is often deeper: at high intensity, the sympathetic nervous system triggers vasoconstriction in the digestive region. Blood partially leaves the intestine to supply the muscles.

In certain situations, intestinal blood flow can drop dramatically. This reduced perfusion causes relative ischemia: the gut receives less oxygen, fewer nutrients, and diminished repair capacity. In the short term, this can cause nausea, cramps, diarrhea, abdominal pain, urgent bowel movements, bloating, or an inability to properly absorb what is ingested during exercise. Over the long term, if stress is repeated without recovery, the intestinal barrier can become more permeable and inflamed.

Cortisol plays a role in this process. During intense or prolonged effort, it helps mobilize energy. It supports glucose production, maintains blood pressure, and prolongs alertness. But elevated cortisol is incompatible with optimal digestion. Digesting requires parasympathetic activation, available blood flow, motility, and a calm mucosa. Performing often demands the opposite: tension, mobilization, and muscular priority.

Insulin is also part of the equation. Before or during exercise, poorly timed carbohydrate intake can cause energy fluctuations, digestive responses, or a feeling of heaviness. During exercise, muscle contraction enables glucose uptake via GLUT4, sometimes with less insulin dependence. But this doesn’t solve everything: if the intestine is poorly perfused, ingested fuel may remain in the digestive tract and become a burden rather than a help.

This is where the carnivore or low-carb approach must be smart. An athlete adapted to fats can better sustain moderate, prolonged efforts with less reliance on frequent carbohydrate intake. This can reduce digestive load during certain efforts. But once intensity climbs very high, glycogen remains a limiting factor. The goal is not to follow dogma but to understand the real demand: intensity, duration, heat, hydration, sodium, training volume, recovery, and individual tolerance.

A common mistake is trying to eat during an effort when the body has already decided digestion is incompatible. The higher the intensity, the more the digestive tract becomes a secondary territory. Forcing gels, sugary drinks, fiber, fats, or proteins at this time can worsen the problem. The intestine doesn’t refuse out of whimsy. It is simply underperfused.

The strategy must therefore be built upstream. Arrive at exercise with a calm digestive terrain. Avoid heavy meals too close to effort. Test intake during training, never on race day. Adjust salt and water. Respect heat conditions. Distinguish between long moderate effort and long intense effort. And above all, accept that extreme performance always comes with a biological hierarchy: everything not essential to immediate action can be temporarily sacrificed.

The real question is not just: “What to eat during exercise?” It is: “At what intensity can my body still digest what I give it?” Until this question is asked, many athletes will continue to treat the gut as a reservoir, while during extreme effort it often becomes the first silent victim of performance.
